The future is here. Your legacy systems don't know it yet.
Agentic technology and superintelligence will redefine how healthcare runs. Elaria Agentic helps C-suite leaders integrate agents into the systems they already own — with the judgment of an operator who has spent 20 years inside S&P 500 companies, hospitals, and academic medicine.
The institution is the patient. Margins are the vital sign.
The median nonprofit hospital now clears about 1.2 to 1.5 percent on operations. Roughly 43 percent lose money on operations. Fee-for-service Medicare pays about 12 percent below cost.

THE DIAGNOSIS
Profit is what great people and a great product earn, minus what broken process leaks. In most hospitals the first two are excellent. The third is where the margin goes.
I evaluate every organization through three buckets — People, Product, Process. I borrowed the lens from The Profit, Marcus Lemonis's turnaround series, because it is the most practical way I have found to break a complex operation into parts a leadership team can actually act on. Twenty years across insurance, healthcare and education have only confirmed it: when one bucket is misaligned, the other two cannot save you.
Hospitals have world-class people and a product patients trust. What they are missing is process: legacy systems, manual handoffs, prior-authorization queues, denials, and skilled clinicians doing clerical work. That is where agentic AI belongs — not replacing Epic or the ERP, but sitting on top of them and closing the leaks.
So I treat the institution the way a clinician treats a patient: history, vitals, diagnosis, treatment plan, follow-up. The vitals are the margin, the payer mix, the denial rate, the days cash on hand. The treatment is a sequenced roadmap a chair, CEO, COO or CFO can defend to a board.
Every engagement starts with your existing systems and ends with a decision your board could approve.
01 — EDUCATION
Executive Agentic Literacy
Half-day and full-day sessions for leadership teams and boards: what agents are, what superintelligence changes, and what it means for your P&L, workforce, and risk posture. No vendor pitch, no jargon.
The history of transitions. Every session opens with a walk through the disruptions that came before — electrification, mainframe to PC, the internet, the EHR and cloud eras — what each did to the organizations living through it, who adapted, who didn't, and why. Agentic AI makes more sense once you can see where it sits in that lineage.
Board and C-suite briefings
Department-chair and service-line workshops
Keynotes and panels
02 — INTEGRATION
Legacy Systems Integration Roadmap
A practical map of where agents belong across your EHR, ERP, supply chain, revenue cycle, and compliance workflows — sequenced by value, risk, and readiness, and built to be approved by procurement and IT security.
A multi-specialty team. Every roadmap is delivered with an IT integration consultant who has hands-on experience connecting legacy systems. The team is insured for healthcare engagements and for non-healthcare engagements alike, so the approach fits your business whichever side of the line it sits on.

03 — OPERATIONS
Agentic Operations Advisory
Fractional, retained guidance for executives standing up their first agent programs: accountability, vendor evaluation, change management, and the metrics that tell you whether it is working.
On governance and risk, a plain word. Agentic AI is largely unregulated today, the vendor field changes monthly, and no one can promise a foolproof framework. What we do is take into account everything that is out there — federal and state activity, payer rules, what peers are doing — add our own pearls from running operations, and give you our honest two cents. You get judgment, not guarantees.

04 — PILOTS
Pilot Design & Oversight
Scope, staff, and supervise a 90-day agent pilot in one function, with clear success criteria and a go/no-go decision at the end. In healthcare that might be supply chain, scheduling or prior authorization. In any business it might be the inbox, invoices, reporting or customer follow-up — the work is the same shape.
Example: the inbox. Our model of a 40-hour administrative role puts 40–60% of the week — 16 to 24 hours — on agents: triage, routine replies, follow-ups, scheduling, copy-paste between systems. A role that is mostly email reaches 60–80%. Illustratively, one email-heavy seat runs about $78K a year today and about $27K with an agent doing the routine share — roughly $50K — and the person moves from doing the tasks to running the system that does them.

Approach
How an engagement runs
1
Listen
We start where the work begins — with the people who actually do it: the scheduler, the coder, the supply tech, the nurse. Then we work up through every layer to the C-suite. The problem described in the boardroom is rarely the problem on the floor.
2
Diagnose
People, Product, Process — mapped layer by layer from what the front line told us, not from an org chart: where each is strong, where it breaks, and where agents fit.
3
Decide
An executive brief with options, costs, and a recommendation — written for a board, not an IT ticket.
4
Deliver
Hands-on oversight of the roadmap or pilot until your team can run it without us.
THE RULE WE DO NOT BREAK
Go to where the work begins. Talk to the people doing it. Then climb the layers.
Most consulting starts at the top and never leaves the conference room. We start at the desk, the dock and the clinic, and we do not write a diagnosis until we have heard the same story from the front line and from every layer between it and the chair. Every agent we design is built around how the work is actually done.
Founder
Miguel Morel
Founder & Chief Agentic Operations Executive
I founded Elaria Agentic because I believe agentic technology and superintelligence will redefine the next decade — and because most of the executives who will live through that shift have never had someone explain it to them in the language of their own operations.
For 20 years I have run operations inside S&P 500 companies, academic medical centers, and education — orthopedics, supply chain, vendor contracting, revenue cycle, compliance, and physician practice management. I know what a legacy system looks like from the inside, because I have been the person responsible for keeping one running.
I hold an MBA and am attending Columbia University's Mailman School of Public Health for an Executive MHA. I look at every organization through three buckets — People, Product, Process — and I believe emotional intelligence is the first tool any leader brings to a transformation.
